WELLNESS EVENT WAIVER, RELEASE & ASSUMPTION OF RISK AGREEMENT

Activities: Breathwork, Mobility Flow, Contrast Therapy, Sauna, Cold Plunge, Hydrotherapy, Biohacking Tools
- VOLUNTARY PARTICIPATION & ASSUMPTION OF RISK
I understand that participation in the above activities involves inherent and potential risks, including but not limited to: slips or falls; exposure to heat or cold; dizziness; fainting; cold shock; elevated heart rate or blood pressure; dehydration; infection; aggravation of pre-existing conditions; serious injury; disability; stroke; heart event; or death. I affirm that I am voluntarily participating, am physically able to do so, and have consulted a physician if I have any medical condition that may be affected by participation. I agree to stop immediately if I feel unwell. I accept full responsibility for my participation and for use or misuse of any equipment or facility areas.
- RELEASE, WAIVER & HOLD HARMLESS
To the fullest extent permitted under Missouri law, I knowingly and voluntarily release, waive, discharge, and hold harmless Amini’s, PT 1-on-1, LLC, Reclaim Your Shine, LLC, Chef Angela, and their respective owners, members, managers, employees, contractors, agents, representatives, and affiliates from any and all claims, demands, causes of action, damages, losses, or liabilities of any kind arising out of or related to my participation, including claims based on ordinary negligence. This release includes, without limitation, claims arising from conditions of the premises (including wet surfaces or uneven areas), equipment use, instruction provided, or participation in any activity. I understand this release does not apply to claims arising from gross negligence or willful misconduct, where prohibited by law.
- MEDICAL ACKNOWLEDGMENT
I understand sauna, hot tub, cold plunge, hydrotherapy, breathwork, and biohacking tools may affect heart rate, blood pressure, circulation, oxygen and carbon dioxide levels. Risks may be increased for individuals with cardiac, respiratory, circulatory, neurological, metabolic conditions, pregnancy, recent surgery, glaucoma/retinal issues, seizure disorders, psychiatric conditions, or open wounds. Cold plunge temperatures will not be set lower than 50°F. I acknowledge these activities are not medical care or treatment. I am responsible for disclosing relevant health conditions and obtaining medical clearance if necessary.

By registering for and attending this event, I authorize the participating hosts (Amini’s, PT 1-on-1, LLC, Reclaim Your Shine, LLC, and Chef Angela) to share and use my contact information (name, email, phone) for event-related and future wellness communications. I understand I may opt out of communications at any time.
- ARBITRATION & VENUE
Any dispute arising from this event shall be resolved by binding arbitration in St. Louis County, Missouri. Missouri law shall govern. Each party shall bear its own costs unless otherwise awarded by the arbitrator.
- NO GUARANTEES
No representations or guarantees are made regarding health outcomes or results. Participation is voluntary and at my own risk.
- MEDIA RELEASE
I grant permission for photographs and recordings taken during the event to be used for promotional purposes without compensation.
